Elgg version: 1.6

The version is not set during installation. This means the first time upgrade.php is called all upgrades are run whether needed or not, possibly leading to corrupted databases (as the case in the UTF8 db upgrade.) Errors from running unnecessary upgrades are now reported because of fixing #1424.

(In [svn:3791]) Fixes #1432: Version number is set during installation.
Refs #1424: The upgrade functions now detect if no version number is saved to the db and silences all upgrade warnings and errors. This is required for all installations < this commit to upgrade correctly because of #1432.
More strict regex for finding upgrade files.
Upgrade mysql.sql schema with latest.
